Serving Carmel, Indianapolis, and central Indiana · Licensed Occupational Therapist

FOR REFERRAL PROFESSIONALS

Your OT Home Assessment Referral Partner in Central Indiana

When a client can only stay home if the home becomes safer, an OT home assessment referral to Living Well gives them a licensed occupational therapist who responds quickly and follows through. I work with hospital social workers, discharge planners, rehab teams, waiver case managers, and in-home care agencies across Hamilton, Marion, Boone, and Hendricks counties.

How I Support Your Clients

Hospital Social Workers and Discharge Planners

Patients going home with new mobility limits need a home that’s ready. I schedule quickly so families can make urgent changes around the discharge date.

Rehab Facility Teams

I carry your therapy goals into the home environment, checking whether the bathroom, entry, and bedroom will support what your patient practiced in rehab.

Indiana Medicaid Waiver Case Managers

I’ve completed waiver home modification assessments since 2023 and deliver thorough documentation that meets state standards.

In-Home Care and Waiver Service Agencies

Modifications that keep clients home longer also protect your caregiving staff from lifting and transfer injuries. I look at how your aides provide care, not only at how the client moves.

A Referral You Can Make with Confidence

Your reputation goes with every referral. You’ll send families to one consistent professional instead of a rotating staff. I’m independent, so I never push products or installations, and families receive a written plan they can act on with any vendor.

How to Refer a Client

Choose whichever method is easiest for you:

What to Include in Your Referral

  • Client’s name, city, and best contact person

  • Primary diagnoses and current mobility equipment

  • Payment source: waiver, Medicare or insurance, or private pay

  • Main concerns, recent falls, or an upcoming discharge date

  • Your name and contact information for follow-up

To protect your client’s privacy, please keep form and email messages to basic details and share health information by phone.

Referral FAQs

How quickly will you contact a referred client?

I reach out personally and promptly, because delays are often what families have struggled with most. Referrals are a priority in my schedule.

Do you coordinate with the client’s care team?

Yes. With the client’s permission, I’m happy to communicate with case managers, therapists, and agency staff so everyone works from the same plan.

Can you assess clients who aren’t on a Medicaid waiver?

Yes. I serve private-pay clients and can walk families through Medicare and insurance questions when they’re unsure how the assessment will be paid for.

Have a Client Who Needs a Safer Home?

One call is all it takes to get the process started.
