Home Modification Assessment Cost and Coverage

Wondering about home modification assessment cost, or who pays for it? The answer depends on which path fits your family: an Indiana Medicaid waiver, Medicare or private insurance, or private pay. This page explains each option in plain language so you can move forward with confidence.

Indiana Medicaid HCBS Waivers

Indiana’s Medicaid Home and Community-Based Services (HCBS) waivers help eligible people receive care at home instead of in a facility. For waiver participants, a home modification assessment is typically covered as part of the process.

Starting with Your Case Manager

In most cases, your waiver case manager makes the referral to Living Well. I complete the in-home assessment and provide documentation that meets state standards.

Clients Age 60 and Older

Indiana now serves most waiver participants age 60 and older through Indiana PathWays for Aging, a managed care program. If your parent is enrolled, start by asking their care coordinator whether a home modification assessment is available through their plan.

What Medicare Does and Doesn’t Cover

Original Medicare generally does not pay for home modifications such as grab bars, ramps, stair rails, or bathroom remodels, and it does not typically cover a standalone home modification assessment.

Medicare Part B may cover occupational therapy when a doctor orders it as medically necessary treatment, and home health benefits can include therapy for people who qualify as homebound. Some Medicare Advantage plans also offer supplemental benefits for in-home safety items. Because the rules vary, call your plan before scheduling and ask specifically about home safety evaluations and modifications.

Private Health Insurance

Coverage for home modification assessments varies widely among private insurers. Some plans cover occupational therapy in the home; most do not pay for the modifications themselves. I’ll tell you which questions to ask your insurer so you can make an informed choice.

Private-Pay Assessment Packages

If you’re paying privately, you choose how much of the home I assess. Every package includes the in-home visit, observation of your loved one’s daily tasks in those areas, and a detailed written report.

Focused Assessment: One Area

Ideal when a single space is the main concern, such as a bathroom that’s become difficult or an entry with steps.

Expanded Assessment: Two to Three Areas

A good fit when trouble spots connect, such as the route from bedroom to bathroom, or the entry plus the kitchen.

Whole-Home Assessment

A complete look at every relevant area, recommended after a fall, a major diagnosis, or a hospital discharge, or when planning to age in place long-term.

Call for current package pricing. Online payment isn’t offered through this site; I’ll explain how payment works when we talk.

Paying for the Modifications Themselves

The assessment and the modifications are separate costs. Once you have my report, you hire whichever contractor or vendor you prefer and can request bids based on the same detailed recommendations. Community programs may help with funding, and I keep a list of resources for families.

Coverage disclaimer: Coverage varies by plan and program. This page offers general information and is not a guarantee of payment. Please confirm benefits with your insurer, your waiver case manager or care coordinator, or Medicare before scheduling.

Cost and Coverage FAQs

How much does a home modification assessment cost, and who pays?

It depends on your path. For Indiana Medicaid waiver participants, the assessment is typically covered through the waiver. Private-pay clients select a package sized to the number of areas assessed. Call (317) 548-6500 for current private-pay pricing.

Will Medicare cover a home modification assessment?

Usually not as a standalone service. Original Medicare does not typically pay for home modifications or assessments for them, though it may cover occupational therapy a doctor orders as medically necessary. Some Medicare Advantage plans offer extra home safety benefits, so check with your plan.

Does Medicaid cover grab bars and other modifications?

For eligible Indiana HCBS waiver participants, modifications such as grab bars, ramps, and bathroom changes may be covered when approved through the waiver. Your case manager can explain your waiver’s specific rules and limits.

Does the assessment fee include the cost of modifications?

No. The assessment covers my visit and written report. Modifications are purchased separately from the contractor or vendor you choose, and because I’m independent, you’re free to compare prices.

Not Sure Which Option Applies to You?
